Use BlackBerry Desktop Software 6.0
Open the software
Plug the usb to the "old" blackberry
Go to "Device" -> "Switch Device"
Follow the wizard instruction
Disconnect the "old" blackberry
Plug the usb to the "new" blackberry
Done.
